Insemination is a technical process where we monitor the ovulation in the patient and at the right time, at the time where actually the ovulation is expected, the sperm cells (either of the partner or the donor) are actually inserted in to the top of the uterine cavity thus avoiding the trip through the cervical canal or whereas mucus with antibodies which represents kind of a tougher part of the trip for the sperm cells so, they get placed closer to the fallopian tubes and closer (hopefully) to the ripe egg thus taking care of some of the obstacles that might be in the way. What are the conditions for this therapeutic protocol: number one – the fallopian tubes must be freely passable, there has to be a regular menstrual cycle and the regular ovulation cycle in the patient with the favorable folliculogenesis (where the follicle reaches the right size and we can support the ovulation) – that’s having a very strong a good chance for the actual ovulation and of course last but not least, a strong spermiogram. It should be a spermogram that is within the current norm according to WHO-5 from the 2010 so the sperm cells have to have a certain concentration, motility and morphology. All of these are discussed during the consultation with the couple. Another part or another factor that might play an important role is the age of the patient. The success rates with a simple single application so a single uterine insemination are not very high. They’re published somewhere between 8 and 10 maybe 10 and 12% but with repeated cycles (that ‘s up to four cycles) using this technique, the success rate might actually reach 25 or according to some sources eating 40% of the success rate. Once again the requirements: a young patient, an ovulation, regular cycle and the passable fallopian tubes and the good spermiogram.
